Access a Copy of Your Medical Records

All patients or their authorised representative are eligible to request access to a copy of their medical records held by this health service. To make application, please read the information sheet What is Release of Information and then complete the application form.

Please send the completed application form together with a copy of two forms of identification and payment of the administration fee to the Clinical Information Department at the hospital where your medical records are held.
