Project: Narrabri VTE Prevention Program

Project Leader: Jocelyn Palmer, Clincal Nurse Educator

Aim: Within 6 months of implementation 80% of adult patients presenting to Narrabri Health Service will have a VTE risk assessment and appropriate prophylaxis completed within 24 hours of presentation.

Project: Fast Track To ESSU

Project Leader: Nicole Ritchie, Emergency Clinical Nurse Specialist

Aim: Within 6 months, 81% of appropriate patients requiring admission to ESSU from the Emergency Department will be transferred within 4 hours of presentation.

Project:  Taking the Risk Out of Transfer

Aim: Within six months 100% of patients transferred between Maitland and Kurri Kurri Hospitals will have a verbal clinical handover and transfer of care documents in place to support transfer of care.

Project: Adverse Drug Reaction (ADR) reporting: getting on track

AIM: To increase the number of ADRs reported to the Calvary Mater Newcastle (CMN) Drug & Therapeutics Committee (DTC) by 2-3 per month within 6 months.
