Aged Care Services available through Inverell Health Service

There are a number of aged care services available through the Inverell District Hospital and Inverell Community Health, each of which are aimed at maintaining the health of people while living in their own homes for as long as possible.

More and more health services are being oriented to prevention, education, screening, early intervention and self management, to minimise the impact of poor health on individuals and reduce the need for people requiring hospitalisation.

Most Community Health services are available without referral.

Community Health services include:

• Aboriginal health – education, screening and referral (Ph: 6721 9635)

• Aged Care Assessment Team (ACAT) – The Northern Tablelands ACAT team provides assessments of older people regarding their level of independence and refer people to appropriate services (eg visiting geriatrician, dementia and respite services, residential aged care)

• Asthma and respiratory education – self management education for chronic lung disease, includes home oxygen services

• Cardiac rehabilitation – education and exercise sessions to people who are rehabilitating after cardiac surgery (referral required from GP)

• Pulmonary Rehabilitation – a multidisciplinary exercise and education program for people with chronic lung disease and their carers

• Community nursing – health assessments, care following discharge from hospital, wound and ulcer management, referrals to other health care providers

• Audiometry – hearing assessment (NB.Pensioners with a hearing aid need to go through the Commonwealth Hearing Service Office Ph: 1800 500 726)

• Dental care – for concession card holders (Call 1300 651 625 for appointment)

• Diabetes education – advice, information and education to aid self management

• Dietitian - education, information and advice associated with disease management and weight control

• Equipment hire - Some medically related equipment and aids are available for short term hire

• Foot care – providing basic foot care

• Podiatry – appointments can be made with visiting podiatrist from Armidale

• Mental health services – includes information and counselling for people experiencing bereavement and depression

• Occupational therapy – assessments for independence, home modifications and equipment

• Oncology – support for people undergoing treatment for cancer

• Palliative care – support for people in the final stages of life, includes bereavement support for family

• Primary health care – assessment, education and referral for families in outlying areas

Inverell Hospital services include:

• Day Care - promoting the psychological, social and physical well being within a group environment with support from Diversional Therapist

• Discharge planning - co-ordinated liaison with doctors, patients and family, specialist staff and community agencies following discharge from hospital

• Palliative care unit – providing quality symptom management for terminally ill patients, and bereavement support to family and friends

• Physiotherapy service - providing continued treatment of injuries and appropriate disorders to inpatient and outpatients

• Rehabilitation service – providing education, information and support to people recovering from strokes, physical disabilities, and post operative care.
