Making diabetes care plans work for you

The Forster/Tuncurry Support Group of Diabetes Australia has invited Hunter New England Health Diabetes Educator Malcolm Flood to speak at their next meeting later this month, to update interested people on changes to services offered under Medicare and other information about diabetes care plans.

Malcolm’s topic is the Diabetes Cycle of Care, which covers what people with diabetes should expect to have checked at least once in every 12month cycle as part of an individualised care plan. These include blood tests, as well as checks with an optometrist and a podiatrist or other health professionals.

He will also cover the recent changes to the Commonwealth Government’s funding for Enhanced Primary Care (EPC) plans. Malcolm describes the EPC as a mix of allied health services recommended by the doctor managing your diabetes care plan, to tailor your care to best meet your individual needs.

“These services are covered under Medicare, so they cost nothing or very little, and can include visits to dietitians, exercise physiologists, and a host of other service providers who are able to provide specialised support as part of your individual care plan,” Malcolm said.

Everyone is welcome to attend the meeting in the Forster CWA Rooms in Little Street on Monday 24 September. Gold coin donations will be appreciated.
