Multicultural Health Liaison Program – Community Health & Aged Care
The Multicultural Health Liaison Officer offers support to both staff and clients to ensure that the aged and disabled members of the culturally and linguistically diverse community across Hunter New England have access to culturally and linguistically appropriate care.
- Weekly visits culturally and linguistically diverse clients in the Rankin Park Rehabilitation Unit to ensure that the procedures for culturally and linguistically diverse clients have been followed, that clients understand diagnosis and treatment and are able to make informed decisions and are satisfied with care.
- Visits to clients in their own home to ensure that the services they receive are culturally appropriate.
The Multicultural Health Liaison Officer takes referrals from HNE Health clinicians, from community members and from workers in non-government agencies.
The Multicultural Health Liaison Officer works with non-government agencies to research service gaps and to develop programs specifically for culturally and linguistically diverse elders.
The Multicultural Health Liaison Officer undertakes research projects to develop an evidence-based practice.
