Rehabilitation Patient Journey: Maitland Hospital

The Rehabilitation Patient Journey Project started on 27 August 2007.

Aim

The project aims to identify and implement a best practice model of care for patients and carers using rehabilitation services at Maitland Hospital.


Next Steps

The 16-week project is made up of four key stages:

1. Planning
Project initiation and planning

2. Start-Up
Designing the detailed project methodology, including plans for communication and consumer engagement.  A best practice model of care will be developed by reviewing the literature and working with rehabilitation experts.

3. Diagnostics
Collection and analysis of data and information to understand gaps in the current situation and to prioritise issues for the solution design phase. 

4. Solution Design
Design of solutions to address the prioritised issues in order to implement the model of care. 


Implementation and Sustainability

Following solution design, a six-month phase will focus on implementation and building sustainability of the proposed models and solutions.


Governance

The Executive Sponsor for the project is Mr Chris Kewley, Director of Nursing and Midwifery.  A Project Reference Group, made up of a clinical steward, other senior managers and the project managers, will advise and oversee the project.

Project and administrative support will be provided through the Innovation and Reform Unit.

The Project Leader (Louise Jordan) and Project Manager (Mary Byrne) are both participants in the newly established NSW Health Centre for Healthcare Redesign.  They will attend the Centre intermittently across the course of the project.
