Referral and Clinical Matters
Patients are admitted to the Out & About Home IV Therapy virtual ward and are managed as inpatients during their care at home. Care may be provided in the home by community nurses or may be patient / carer administered treatments after education by the Out & About Home IV Therapy team.
Adult and peadiatric referrals are accepted for the following conditions:

Bone & Joint Infections (e.g. Osteomyelitis, Septic Arthritis, Infected Joint Replacements or Hardware);

Infected Devices / Implants e.g. Pacemaker, Vascular Grafts, Implantable Ports

Infective Endocarditis
-
Abscess e.g. Cerebral, Psoas, Lung, Liver/Splenic;
-
Cystic Fibrosis
-
Septicaemia / Bacteraemia
-
Bronchiectasis
-
Empyema
-
Pyelonephritis
-
Cellulitis
-
Recurrent dehydration
-
Multiple other conditions requiring intravenous therapy (please enquire)
Patients may be prescribed single or multiple intravenous medications through one or more modes of administration e.g. continuous, intermittent and bolus.
Hours of operation are 7am – 5.00pm 7 days, with 24 hour on-call nursing and medical staff. Patients have a weekly medical review in the Out & About Home IV Therapy clinic to monitor their progress and modify treatment if required.
Refer patients early so that plans can be made for admission to the Out & About Home IV Therapy program.
Once the patient is referred, changes in the patient’s condition or treatment should be communicated to the Out & About Home IV Therapy team, so that adjustments to discharge dates and medication orders can be made.
- Patients for referral to the Out & About Home IV Therapy Program should first be discussed with the Infectious Diseases registrar (page 5630) or Dr Mark Loewenthal through JHH switchboard (02) 4921 3000.
- Complete the Referral Form and fax to 49223468.
Catchment area - patients are accepted from the Hunter region extending from Morisset in the south to Buladelah in the North and Murrurundi in the North West. Occasionally patients from outside this geographic boundary can be admitted to the program.
