Referrals and Clinical Matters

The service promotes early discharge of patients following their acute admission from Manning Hospital and offers an alernative to inpatient care.  A rapid response (same day service) is available.  Hospital in the Home operates 7 days a week from 7am to 9pm.  Catchment Area includes Taree, Wingham, Tinonee, Old Bar, Nabiac, Harrington, Lansdowne, Forster / Tuncurry and Hallidays Point.

Services provide:

  • Anticoagulation
  • Intravenous Antibiotics
  • Baxter Infusions
  • Care of Central Venous Catheters
  • Post Operative Care

Referrals are accepted for the following conditions:

  • Deep Vein Thrombosis
  • Pulmonary Embolus
  • Atrial Fibrillation
  • Cellulitis
  • Post Operative Joint Replacements
  • Heart Failure
  • COPD
  • UTI

Eligible Patients:

  • Manning Hospital Inpatients
  • Outpatients assessed in Emergency Department
  • Lives within approximately 30 minute drive from Taree Community Health
  • Patient for IV therapy must have a carer
  • Must be over 16 years of age

All patient will be assessed by Hospital in the Home staff prior to acceptance to the program.
