Referrals and Clinical Matters
The service promotes early discharge of patients following their acute admission from Manning Hospital and offers an alernative to inpatient care. A rapid response (same day service) is available. Hospital in the Home operates 7 days a week from 7am to 9pm. Catchment Area includes Taree, Wingham, Tinonee, Old Bar, Nabiac, Harrington, Lansdowne, Forster / Tuncurry and Hallidays Point.

- Anticoagulation
- Intravenous Antibiotics
- Baxter Infusions
- Care of Central Venous Catheters
- Post Operative Care
Referrals are accepted for the following conditions:
- Deep Vein Thrombosis
- Pulmonary Embolus
- Atrial Fibrillation
- Cellulitis
- Post Operative Joint Replacements
- Heart Failure
- COPD
- UTI
Eligible Patients:
- Manning Hospital Inpatients
- Outpatients assessed in Emergency Department
- Lives within approximately 30 minute drive from Taree Community Health
- Patient for IV therapy must have a carer
- Must be over 16 years of age
All patient will be assessed by Hospital in the Home staff prior to acceptance to the program.
