Hospital in the Home Greater Newcastle

This service provides acute care to clients who require ongoing nursing care as well as medical and or allied health interventions. Our mission is to focus on hospital admission avoidance and or facilitation or early discharge. Clients must have otherwise required admission or have an anticpated prolonged hospital stay if Hospital in the Home was not available, or be considered at high risk for representation to hospital.
The multidisciplinary health team includes a staff specialist medical director, a medical registra, physiotherapists, occupational therapist, dietician and nursing staff.
To be eligible for Hospital in the Home clients must:
- be assessed by Hospital in the Home staff prior to discharge
- be over 16 years of age
- have a suitable diagnosis and ongoing care needs with appropriate diagnostic tests performed
- be clinically stable
- if not self caring, have a suitable carer willing to participate in care management
- have a home / facility risk assessment attended
- be within our catchment area of the local government areas of Newcastle, Lake Macquarie and Port Stephens.
The types of treatments provided include, but are not limited to:
- Anticoagulation
- Short term intravenous antibiotics
- Early discharge post orthopaedic surgery
- Education and monitoring
- Functional assessment / management
The patient may have:
- Acute infections e.g.
- Cellulitis
- Pyelonephritis
- Chronic Obstructive Pulmonary Disease
- Chronic Heart Failure
- Thrombo-Embolic Disorder
- Deep Vein Thrombosis
- Pulmonary Embolus
- Transient Ischaemic Attack (TIA)
- Ischaemic Stroke
- Atrial Fibrillation
- Falls and or mobility issues
Clients with other conditions or treatments requirements can be considered on a case-by-case basis following consultation with Hosptial in the Home staff.
