Hospital in the Home
Hospital in the Home is a service that provides treatment in the patient's home for a condition that otherwise would require acute hospital in-patient care Hospital in the Home services differs from Community Health services as they provide the patient with acute treatment. Hosptial in the Home staff members can include doctors, nurses and allied health staff. CAPAC, Community Acute / Post Acute Care
The types of treatments provided include, but are not limited to:
- Anticoagulation
- Short term intravenous antibiotics
- Early discharge post orthopaedic surgery
- Education and monitoring
- Functional assessment / management
The patient may have:
- Acute infections e.g.
- Cellulitis
- Pyelonephritis
- Chronic Obstructive Pulmonary disease
- Chronic Heart Failure
- Thrombo-embolic disorder
- Deep Vein Thrombosis
- Pulmonary embolus
- Trans-ichaemic attack
- Cerebrovascular accident
- Atrial Fibrillation
- Falls and or mobility issues
