- Transitional Aged Care Program - Singleton (Seven days, 8am-4pm)
- 12 week post hospital therapy program - All client must have ACAT assessment before leaving hospital and be assessed by TACP before discharge - Referral 0434 361 425

Specialised Nursing
- Palliative Care (8.30am-5pm, seven days per week)
- Based at Cessnock Community Health. Provide specialised palliative nursing and supportive care services across the Cessnock and Singleton LGA's in partnership with GP's, Silver Chain nurses, community and hospital nurses, allied health; Specialist Medical; housekeeping staff and trained volunteers.
