The advance care planning DVD- A Family's Journey is a 15 minute DVD that demonstrates why advance care planning is of benefit to individuals and their families and how individulas can make advance care plans for themselves. The DVD shows one family having advance care planning discussions and how these conversations can help doctors and families make decisions when a loved one loses their decision making capacity.
The DVD was made to introduce the concept of advance care planning to the community. The DVD is suiitable for personal, group and health care professional use. It has been effectively used for chronic condition education groups and carer education groups.
