Advance Care Planning For Health Care Providers
Advance Care Planning means that the health care delivered to our patients is:
- person-centred
- encourages and supports increased patient understanding of their illness and treatment
- enables the patient to be involved in planning their treatment.
As health care professionals, we can assist people with their Advance Care Planning needs. We already do. We make sure that they understand their health, and offer them information and support as they need it.
One fundamental part that we can improve upon is ensuring that a person knows who their substitute decision maker would be in the event that they lose the capacity to understand, make or communicate their medical decisions. Asking this question when a patient is admitted to our care, then offering information about Advance Care Planning, equips them to start Advance Care Planning for themselves.
