Each time they are amended the form version numbers will change. Applications submitted on outdated forms will not be accepted.

For multi-centre research the submission to a Lead HREC must be on the National Ethics Application form and this form must be completed online via http://www.ethicsform.org/au as this version of the form interfaces with the Research Ethics Database (AU RED) which is a web-based research tracking and management mechanism that will underpin the new system. Single-site research that is, research to be conducted only within Hunter New England Health) may be submitted either on the National Ethics Application Form online via http://www.ethicsform.org/au r the Hunter New England Human Research Ethics Committee application form (Version June 2007).
* The Guidelines must be read before completing the application form. Twenty-five copies of the application form and an electronic copy should be submitted by email to hnehrec@hnehealth.nsw.gov.au. For further information about submitting an application please see the front page of the initial application form. The Site-Specific Assessment Application Form should be submitted for all projects submitted to the Hunter New England Human Research Ethics Committee and also for all projects to be conducted at a Hunter New England Health Site but approved by another lead Human Research Committee under the system for single ethics and scientific review multicentre research within NSW Health. For multicentre research this form must be completed online via http://www.ethicsform.org/au. For Single-site Research, the Site-Specific Assessment Form should also be submitted on-line if the ethius application has been submitted on the NEAF or it may be downloaded for ethic applications submitted on the Hunter New England Human Research Ethics Committee’s a
Renewal - The Renewal application is to be used to renew approval for existing projects which will extend beyond the standard three-year approval period or the extended five-year approval period. You will be advised if you are required to submit a Renewal Application.
Four copies of the Renewal application form and any other attachments should be submitted. For further information please see the Renewal application form. Variation - The Variation application is to be used for requesting approval for proposed variations or amendments to research protocols which have an existing human ethics approval.
Two copies of the Variation application form and any attachments should be submitted. An electronic copy of the application for Variation and all the attachments should also be submitted to hnehrec@hnehealth.nsw.gov.au. For further information please see the Variation Application form |
